Can synovectomy codes (29875 / 29876) be reported alongside other arthroscopic knee procedures on the same knee? Under what conditions?
CPT 29875 should not be reported together with another arthroscopic procedure on the same knee (ipsilateral side) if it overlaps in compartments. It is considered part of the primary service unless separated by compartment or distinct medical necessity.
CPT 29876 can sometimes be reported with other procedures, but only if the synovectomy is in compartments not involved in those other procedures, so there is no overlap.
When it is valid, modifier -59 (or other appropriate modifiers per NCCI) may be needed to indicate that the procedures are distinct. Documentation must support the distinctness.
